The requesting agency should provide the Live Scan Request form. The requesting individual or agency is responsible for completing the sections relating to the agency information.
• Applicant should bring the completed Live Scan Request Forms
• Valid United States issued photo Identification Card
• Payment: Cash, Credit Card, and Agency Billing Number
• Be on time
